Most of the scientific literature concludes that saffron does not increase blood pressure, and thus the use of saffron-containing products may protect against cardiovascular disease and/or high blood pressure. However, the scientific literature on saffron has not been fully evaluated in a randomized controlled trial.
Most published articles and clinical trials on high-saffron foods and nutritional value cite either results from single-blind cross-over or cross-over designs which are known to be imprecise and confounded. In a systematic review on the nutritional value and cardiovascular risk factors in men and women, Bijlevensis et al discovered substantial differences between healthy volunteers (n=45) who ingested varying amounts of high-calorie, high-fiber, and high-saffron foods, and those who consumed the same amount of low-calorie, low-fiber, low-saffron foods. There were statistically significant differences between the groups by total energy intake (mean: 44.3±15.3 vs 48.2±16.5 kcal/day; P<0.02, Figure 5). As shown in Table 3, one-third of the low-fiber group and one-half of the high-calorie group consumed significantly more high-fiber saffron food (p < 0.01). However, the high-fiber group consumed, on average, a significant higher fiber (0.45±0.08 g/day vs 0.37±0.05 g/day), greater total and fiber-to-total fat ratio compared to the low-fiber group (1.33±0.01 vs 1.15±0.09 g/day, respectively; p=0.011). In the review, there was also a suggestion that, "when high-quality saffron food is consumed, there is no increase in blood pressure among postmenopausal women and no increase in fasting blood pressure among persons with hypertension". However, this statement was criticized because the small sample size of the study may cause a lack of statistical power.
